Standard care after a thyroidectomy checks one number: TSH. It’s adjusted until it sits in range, and that’s treated as the finish line. But TSH is an indirect signal: it doesn’t show your Free T3 (the active hormone your cells actually use), how well your body is converting your medication into that active form, or your nutrient status. None of that shows up on a standard thyroid panel.
A healthy thyroid makes two hormones and releases some active T3 directly. After a total thyroidectomy, that’s gone. Every bit of active hormone your body uses now depends on converting a single input (your daily medication), and that conversion needs the right nutritional cofactors to run well.
Research published in PLOS One (Gullo et al., 2011) looked specifically at people with no functioning thyroid taking thyroid hormone replacement, and found that even with a normal TSH, they tended to have lower Free T3 than people with an intact thyroid. A normal TSH doesn’t always mean a normal supply of active hormone reaching your cells.
