Coffee and Thyroid Medication: How Long Should You Wait?

Take your thyroid tablet with a glass of water, then wait at least 30 minutes, and ideally 60, before your coffee. That is the same gap the Australian label gives for food, and the one study that timed coffee found that waiting an hour removed its effect. You do not need to give up coffee. You just need to give your tablet a head start, the same way every day.
This guide is for anyone on levothyroxine, whether for Hashimoto's, an underactive thyroid or life after thyroid surgery. It is general information to take to your doctor or pharmacist.
Why coffee matters at all
Levothyroxine is absorbed best on an empty stomach, and anything in your gut at the same time can get in the way. Coffee turns out to be one of those things. In a small Italian study, taking levothyroxine with espresso-style coffee instead of water reduced how much was absorbed by roughly a quarter to a third. When the same people took their tablet with water and had their coffee 60 minutes later, absorption was similar to water alone.
It is a small study, done with strong espresso, so the exact numbers may not apply to every cup. But the direction is consistent with what reviewers have found, and the fix is simple: tablet first, coffee later.
What the label actually says
The Australian medicine information for levothyroxine says to take it on an empty stomach, at least 30 minutes and preferably 60 minutes before any food or other medications. It does not mention coffee by name, and neither does the US label. So think of coffee the same way as breakfast.
The American Thyroid Association ranks the options from best to worst: 60 minutes before breakfast, then at bedtime, then 30 minutes before breakfast, then with breakfast. It says a 30-minute routine kept the same every day "may also be reasonable".
No study has tested coffee at 30 minutes, so 60 minutes is the safer goal if you can manage it. If 30 is what fits your life, the key is to do it the same way every day.
Milk, soy milk and other drinks
- Milk. In a small study, drinking milk at the same time as levothyroxine lowered absorption by about 8%. Another reason to keep your latte for after the gap.
- Soy milk. Soy is listed on the levothyroxine label as something that can reduce absorption, so the same gap applies.
- Tea. The evidence is thin, but it is reasonable to handle tea the same way as coffee.
- Grapefruit juice. It had only a small effect in one study, but it still counts as more than water.
- Water. Plain water is the only drink to take your tablet with.
The real rule is consistency
Your dose is adjusted against your routine, based on your tests. If you always have coffee 30 minutes after your tablet, your dose has quietly been set with that in mind. The biggest risk comes from changing the routine, not from the routine itself.
So whatever your gap is, keep it the same. If you change it, for example by moving coffee from straight after your tablet to an hour later, your absorption may improve. Mention it to your doctor so they can decide whether to recheck your thyroid.
If mornings just do not work
Some people find the morning gap impossible. Taking levothyroxine at bedtime, on an empty stomach at least three hours after your evening meal, is a recognised alternative. In a trial of 90 people, bedtime dosing raised thyroid hormone levels slightly on paper, though people did not feel any different. It is a change to discuss with your doctor first, not one to make on your own, because your dose may need checking.
Coffee and how you feel
Coffee can cause jitters, a racing heart or poorer sleep in some people. In a recent trial, days with coffee meant more extra heartbeats and about 36 minutes less sleep. Australia's healthdirect says healthy adults can safely have around 400 mg of caffeine a day, roughly two espressos or four instant coffees.
There is one thing worth knowing if you take thyroid medication. Jitters, a racing heart and poor sleep are also listed signs that a thyroid dose may be too high. If they are new, or do not settle when you cut back on coffee, tell your doctor rather than assuming it is the coffee.
Coffee and iron
Low iron is common alongside Hashimoto's and an underactive thyroid, and coffee affects iron too. In one study, a cup of coffee with a meal reduced iron absorption by 39%. Coffee an hour before the meal made no difference, but coffee straight after it had the full effect. If you take an iron supplement, or are trying to lift your iron through food, keep coffee away from it as well. Iron also needs its own gap from your thyroid tablet, which our guide to supplements and levothyroxine timing explains.
A simple morning that works
- Wake up and take your tablet with a glass of water.
- Shower, get ready, walk the dog: anything but eating or drinking.
- After at least 30 minutes, ideally 60, have your coffee and breakfast.
- Keep any iron, calcium or mineral supplements for later in the day, well away from your tablet.
- Keep it the same every day.
Where ThyroBase fits
If you take your thyroid tablet in the morning, the ThyroBase AM serve goes at least four hours after it, because it contains minerals, so it never competes with your tablet or your first coffee. It has no added caffeine. It is designed to sit alongside your thyroid medication, never instead of it. Here is who it is, and is not, designed for.
You do not have to choose between your tablet and your coffee.
Give the tablet a head start, then enjoy the cup.
Frequently asked questions
How long after levothyroxine can I drink coffee?
Wait at least 30 minutes, and ideally 60. The Australian label says to take levothyroxine at least 30 minutes and preferably 60 minutes before food or other medications, and the one study that timed coffee found that waiting an hour removed its effect on absorption.
Does coffee stop levothyroxine working?
Coffee taken at the same time can reduce how much levothyroxine you absorb. In a small study, taking the tablet with espresso-style coffee reduced absorption by roughly a quarter to a third. Waiting an hour before coffee removed the effect.
Can I take my thyroid tablet with coffee if I always do it the same way?
Your dose is set against your usual routine, so a consistent habit matters most. But taking it with coffee reduces absorption, so the better routine is tablet with water first, then coffee at least 30, ideally 60, minutes later. If you change your routine, let your doctor know so they can decide whether to recheck your thyroid.
Is milk in coffee a problem for thyroid medication?
Milk taken at the same time as levothyroxine reduced absorption by about 8% in a small study. If you have your coffee after the 30 to 60 minute gap, the milk in it is not a problem.
The honest takeaway
Coffee and thyroid medication can share a morning, just not the same moment. Take your tablet with water, give it at least 30 and ideally 60 minutes, then have your coffee. Keep the routine the same every day, keep minerals and iron for later, and tell your doctor if you change things or feel jittery in a new way. Your morning cup is safe.
